Yes. We can file your insurance with Medicaid.

Medicare will not be accepted by most hospitals.

1. Must complete your Electronic Medical History Form within 30 days of receiving it.

2. Must email us your primary care doctor's name and fax number.

3. Make an appointment with your primary care doctor for a history and physical evaluation

-Note signed history and physicals are valid for only 30 dates. Make sure to lock your surgical date down, before having the form signed.

4. Sign Electronic consents and show up for your surgical appointment.

• If you dont show up we wont reschedule you.

Medicaid is considered secondary payer. You will need to contact your private insurance and ask them for prior approval. Once the private insurance denies the request for services, we can bill your medicaid. You will need to provide us with denial letter from your insurance company.

Personal Concierge service / NCHD will dedicate one personal coordinater to work with you during the entire process.
